A year in the life of a Pharmacist's log

• Jan - achievement: Identified interaction between warfarin and new azithromycin in Mr Patel's repeat, caught at till before dispensing. Flagged to GP, adjusted INR monitoring plan. Prevented potential bleed risk.
• Mar - feedback: Anita said in the stockroom: 'Your notes on the metformin switch saved me time with the next patient, really clear.' First time she's asked me to do that proactively.
• Mar - task: Took on all MUR scheduling after Sarah's maternity started. 12 reviews per week, rescheduling no-shows same day. First month messy but process tightened by week 3.
• Apr - achievement: Completed 47 medication reviews in May; 8 flagged dosing issues sent to GPs, 5 resulted in changes. Three patients de-escalated off PPIs after review.
• Jun - task: COSHH audit came back with labelling gaps in our cytotoxic fridge. Spent June weekend relabelling with batch dates and stability info on every bottle. Audit re-check passed.
• Jun - feedback: Manager said in appraisal meeting: 'The cytotoxic compliance work was thorough and you didn't wait to be asked.' Gave me lead on quarterly compliance checks going forward.
• Aug - task: Dispensed a morphine liquid for paediatric patient. Double-checked concentration mid-pour, caught I had drawn 2ml not 0.5ml. Rebuilt the process with visual flags on paediatric bottles.
• Sep - achievement: Ran flu clinic first time this year. 156 patients vaccinated over 3 weeks, zero adverse events, zero wastage. Admin time cut by 20% using new checklist I designed.
• Sep - achievement: Reviewed all statins on chronic disease register. Found 18 patients under-dosed; wrote recommendations to GPs with risk ratios included. 14 GPs agreed to increase doses.
• Nov - task: New EPS system rolled out mid-November. Pharmacy threw it back week one, too many timeouts. Reported specific error codes to IT, helped other staff debug their logins.
• Dec - achievement: Completed level 2 independent prescribing qualification. Licensed to prescribe for minor ailments and minor injuries. Started taking repeat hypertension reviews independently from December 15th.

This year I've strengthened my clinical safety practice across prescribing and compliance. Early intervention on the warfarin-azithromycin interaction and the paediatric morphine catch both reinforced my attention to detail at the point of dispensing; I've rebuilt processes around both to prevent similar gaps. My medication review work scaled to 47 patients in May alone, with 8 dosing issues escalated and 5 resulting in GP changes, and I've since worked through the chronic disease statin register to identify 18 under-dosed patients, of which 14 led to dose increases. I completed my independent prescribing qualification in December and am now handling repeat hypertension reviews autonomously. On compliance, I identified and resolved all cytotoxic labelling gaps ahead of our audit re-check, and the manager has asked me to lead quarterly checks going forward.
